Aged Care GP Panels Initiative
The Aged Care GP Panels Initiative aims to ensure better access to primary medical care for residents of aged care facilities and to enable GPs and allied health service providers to work with homes on quality improvement strategies for the care of all residents.
What is a Panel
A Panel is a list or group of GPs and allied health service providers who have agreed to undertake priority activities. These activities have been locally determined by the division in partnership with the RACFs, GPs and allied health service providers in line with the program guidelines. The work should complement and build on successful activities already being undertaken in RACFs in the division, or assist with the development of new activities. These activities might include:
  • addressing issues of residential access to GPs;
  • participating in RACF’s quality improvement activities;
  • participating in preventative health activities such as fall prevention education;
  • sourcing or adapting protocols that assist in improving medical care for residents and streamlining GP activities;
  • developing strategies to address barriers to primary care service provision; and
  • supporting and working with allied health service providers to enable GPs to facilitate the provision of medical care and build multidisciplinary teams.
The following templates have been developed by the Eyre Peninsula Division of General Practice.
