| The Division supports Chronic Disease Management though: |
- The Australian Primary Care Collaboratives (APCC), a project that aims to improve outcomes for patients with diabetes and coronary heart disease;
- The Primary Care Integration Project (PCIP) that aims to improve communication and cooperation between GPs and other health providers so that patients get the most appropriate care from the most appropriate health care provider; and
- Supporting practices to improve their patient information systems.
